Certified Community Behavioral Health Clinic (CCBHC) helps provide comprehensive, coordinated mental health and substance use services to the individual. CCBHC utilizes an integrated person, and family centered treatment plan to help ensure the individuals needs are being met. CCBHC offers access to care for any individual seeking help for mental health, substance use, or community support regardless of diagnosis, residence, or ability to pay. The goal of CCBHC is to provide a one stop shop for behavioral health services to the individual.

SUMMARY DESCRIPTION OF WORK
Professionals at this level are experienced and able to work with a minimum of supervision. Educational requirements are very specific. Decision making, problem solving, and highly developed interpersonal skills are critical. This position will work within a team environment.

MINIMUM QUALIFICATIONS (one or more of the following qualifications to be met):
- Bachelor’s Degree in psychology, social work, nursing, education) and a minimum of two years of experience in behavioral or mental health services.
- High School Diploma and three years of experience within the human services field
Essential Duties and Responsibilities
- Supervise, hire, and train on-call staff to serve as triage for incoming Community-Based Crisis calls.
- Responsible for tracking staff training, time, and FTO.
- Manage payroll of on-call staff.
- Stay up to date on the crisis care continuum in Iowa and Nebraska by continued education and participation of meeting and networking opportunities.
- Provides triage and crisis intervention of clients for referral sources such as community members, law enforcement, or school systems.
- Assesses client needs and makes recommendations and referrals to the appropriate levels of care.
- Establishes an intervention plan, with client and referral source participation, which specifies immediate treatment needs and follow-up.
- Provide case management services as needed to fill in.
- Input referrals via EHR system for responders and clinicians.
- Maintains relationships with key community resources by consistent networking and outreach, as well as provide referrals to clients.
- Ability to handle complex cases.
- Ability to smoothly communicate with responders, clinicians and referral sources.
- Assumes responsibility to be on-call to triage, Monday through Friday. Hours include availability from 8 AM to 8 AM on weekdays, the entire weekend, and holidays. On-call requires answering and triaging crisis calls to determine most appropriate response modality/referral.
- Is dependable and punctual regarding scheduling and attendance.
- Tracks and maintains call data to support continued community-based crisis work.
- Ability to be proficient in Microsoft Office to adequately track and monitor call volume and ongoing staff scheduling
- Identify barriers and gaps in services and works collaboratively with Program Director to improve service areas.
- Updates and maintains a complete on-call triage schedule and covers when necessary.
- Refers to other Heartland Family Service CCBHC programs and other community services, as necessary either concurrent with services or for aftercare.
- Maintains clinical records, meeting established standards of timeliness and completeness.
- Is prompt and accurate in completing all required reports and other requirements.
- Displays a courteous and caring attitude at all times to the clientele, volunteers, and visitors of the Agency.
- Manages and oversees activity in call center platforms.
- Cooperates and collaborates with program area staff, volunteers, and other Agency staff.
- Abides by all specific program and Agency procedures, policies, and requirements.
- Develops personal and program related skills through participation in internal and external training opportunities including printed material and audio and/or visual media.
- *Creates, maintains, and shares an appropriate and dynamic self-care plan.
- *Strives to make connections between the agency and the larger community whenever possible to contribute to the agency’s ongoing fundraising and friend-raising efforts.
- *Essential functions of this job are to be performed remotely from home on the HFS remote worksite.
- Performs other program related duties as assigned.
