SUMMARY DESCRIPTION OF WORK

Respond to clients who may have intensive behavioral health needs in the community.  The purpose of this position is to reach out to individuals in the community, screen for needs, build rapport, connect to appropriate resources, and to provide intensive case management for a small caseload. Highly developed de-escalation, interpersonal and screening skills are critical, in addition to excellent problem solving and decision-making skills. This individual will have a high level of detail-orientation and a willingness work with individuals with complex needs. Specific project, program or office responsibilities may occur at this level.  

Compensation: between $20.92 and $24.96 per hour (wage is based on total years of relevant experience)
Work Schedule: Monday through Friday, 37.5 hours per week, 8:30 a.m. to 5:00 p.m.

MINIMUM QUALIFICATIONS

  • Bachelor’s Degree in Counseling, Social Work or related field preferred
  • Three years’ experience with similar population preferred
  • Valid Driver’s license, acceptable driving record.

Essential Duties and Responsibilities 

  • *Participate in community outreach, screen clients for presenting problem including mental health, substance use, physical health, housing, and other needs.
  • Utilize a strengths-based approach when providing immediate community behavioral health intervention. Ability to model calm and appropriate affect while demonstrating a trauma informed approach to de-escalation that includes empathy and compassion. 
  • *Assist the client in identifying the service options available to address their needs s and make appropriate referrals to those programs, has an extensive knowledge of community resources to work side by side with clients towards linking to programs and meeting service needs.
  • Interface effectively and advocate for clients with other HFS programs, police, court, hospitals, and other community stakeholders and systems.
  • *Assist the client in problem solving any barriers involved preventing the pursuit of any treatment options, including benefit management needs.
  • Demonstrates understanding of culture of homelessness and poverty as well as understanding of substance use disorders and severe and persistent mental illness and associated symptoms.
  • Check client benefits with insurance companies.
  • *Works with clients to ensure Medicaid/insurance coverage and client understanding of their coverage.
  • *Enter collected data into computer database, schedule intake/follow-up appointments and update information as
  • *Complete and process "Release of Information" forms to obtain necessary clinical and other data significant to a client's
  • *Maintain a system of records and reports necessary for statistical documentation of referrals and the tracking of clients from initial inquiry through admission to agency, or advisement of appropriate external resources. Prepare monthly statistical

  • *Display a comfort level in working with stressful individual/family dynamics.
  • *Displays a courteous and caring attitude at all times to the clientele, volunteers, and visitors of the agency.
  • *Cooperates and collaborates with program area staff, volunteers, and other Agency staff.
  • *Is dependable and punctual regarding scheduling and attendance.
  • Abides by all specific program and Agency procedures, policies, and requirements.
  • Develops personal and program related skills through participation in internal and external
  • training opportunities including printed material and audio and/or visual media.
  • *Creates, maintains and shares as appropriate a dynamic self-care plan.
  • *Strives to make connections between the agency and the larger community whenever possible in order to contribute to the agency’s ongoing fundraising and friend-raising efforts.
  • *Essential functions of this job is to be performed on company physical work site.
  • *Performs other program related duties as assigned.
