Job Purpose: Provide direct client services to pregnant and postpartum individuals, helping to improve the health outcomes of children and families throughout Firefly's service area.

Duties and Responsibilities

  • Conduct home health visits to provide preventative education, assessment, and support to prenatal and postnatal clients
  • Develop, implement, and evaluate individualized care plans and provide case management as needed
  • Educate the public about available services and assist with outreach and recruitment activities
  • · Document all client encounters in required databases and maintain timely, accurate records.
  • Assist the Director of Public Health Services with documentation review to ensure compliance with Iowa Health and Human Services and Iowa Medicaid standards.
  • Build and maintain trusting relationships with families, caregivers, and community partners.
  • Travel regularly throughout Southwest Iowa to deliver services and support community programs.
  • Promote the mission and values of Firefly in all interactions and activities.

Additional Responsibilities

  • Support other Public Health functions, including community outreach and Mobile Wellness Unit (MWU) events.
  • Maintain confidentiality of clients and team members at all times.
  • Communicate clearly and effectively, both verbally and in writing.
  • Work flexible hours, including occasional evenings and weekends as scheduled.
  • Continuously seek opportunities for professional growth and development.

Qualifications

  • Registered Nurse (RN) with an active license valid in the state of Iowa (or compact state license)
  • CPR/First Aid certification
  • Knowledge of community resources.
  • Experience providing nursing care in a home visitation setting
  • Valid driver's license; when operating a personal vehicle for company use, must provide proof of adequate insurance
  • Excellent organizational skills with the ability to multi-task and work under pressure
  • Proficiency with Microsoft Office products and electronic documentation systems
  • Bilingual (English/Spanish) preferred (additional $2 per hour for bilingual skills)

Bonus points if you have…

  • One or more years of nursing experience.
  • Experience with providing nursing care to both the child and adult.
  • Experience providing nursing care in a home visitation setting..

Physical requirements

While performing the duties of this job, the employee will frequently sit and stand. The employee is occasionally required to stoop, kneel, and reach forward and above the head. The employee regularly interacts with staff and public in addition to meeting multiple demands and deadlines from several people. The employee must be able to move from place to place, building to building and travel to attend meetings.

Work Environment

  • Work is primarily performed indoors with variable noise levels.
  • Frequent travel throughout the region for home visits and community events.
  • Flexibility in schedule based on client needs and program activities.
